These Terms of Service ("Terms") govern your access to and use of the Health Gauge website, Member Portal, online scheduling tools, and related services (collectively, the "Services"). By accessing or using our Services, you agree to be bound by these Terms. If you do not agree, please do not use our Services.
Health Gauge provides preventive health screening services, health risk assessments, laboratory testing coordination, imaging services, secure result reporting, and related support services for participating employers, unions, Health & Welfare Funds, and individual participants.
You must be at least 18 years of age, or have the permission of a parent or legal guardian where permitted by law, to use our Services.
Certain services may only be available to eligible participants through their employer, union, Health & Welfare Fund, or other sponsoring organization.

You are responsible for maintaining the confidentiality of your username and password and for all activity occurring under your account.
Notify Health Gauge immediately if you believe your account has been accessed without authorization.
Health Gauge provides preventive health screening services designed to identify potential health risks. These screenings are intended to support preventive healthcare and are not a substitute for routine medical care, diagnosis, treatment, or emergency services.
Participants are encouraged to discuss screening results with their personal healthcare provider.
If a screening identifies a critical or potentially urgent finding, Health Gauge may attempt to contact you using the contact information you have provided and may recommend immediate medical evaluation.
Health Gauge does not establish a physician-patient relationship through the use of this website or Member Portal.
Information provided through the Services is intended for educational and informational purposes only and should not be relied upon as a diagnosis or treatment plan.
If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.
Appointments may be scheduled, rescheduled, or canceled through the Member Portal, by telephone, or as otherwise instructed.
Availability is limited and appointments are generally scheduled on a first-come, first-served basis.
Health Gauge reserves the right to modify schedules, locations, or appointment availability when necessary.
